A Clinical Study on Pediatric Abdominal Surgery


Abstract
The author performed a clinical analysis on 147 cases of pediatric abdominal surgery under age of 14. The clinical material was colleced in the Department of Surgery, Seoul Red Cross Hospital from Jan. 1989 to Dec. 1991.
@ES The following results were obtained:
@EN 1) The average age of abdominal operations was at the age of 10.1 and the change of main disease distribution was obtained according to the each age group; the most common disease was gastro-intestinal obstruction in preschool age and
appendicitis
in school age group(p<0.01).
2) The male to female ratio was 1.8:1
3) Common diseases were acute appendicitis(79.6%), intussusception(10.2%), abdominal trauma(4.8%) and congenital anomaly(3.4%) in order of frequency.
4) The perforation rate of acute appendicitis was 29.9% and especially under the age of 7, was 45.8%(p<0.01).
5) The incidence of intussusception was 86.7% under the age of 2, there was no leading point in al cases and the rate of spontaneous reduction was 20% at the time of laparotomy.
6) Postoperative complication occurred in 20 cases(13.6%) and the wound infection was the most common(80%). The perforative appendicitis was apt to have more wound complication (45.7%) than non-perforative one(p<0.01).
7) The bacterial cultures from the infected wound sites showed E. coli most likely and the positive culture rate was 43.8%(7 from 16 cultures).
8) The cases of operative motality was none.
